Co-morbidities: Diabetes/CVD & Mental Illness

The Global Health Institute, Sydney West Area Health Service, invited participants in a planning workshop on "Co-morbidities: Diabetes/CVD and Mental Illness".
The workshop aims to bring together sets of clinicians whose specialist areas dont normally overlap, in order to discover a way of addressing the services and systems issues in dealing with these co-morbidities. Please note that this will be a collaborative planning session, and not a presentation, for people who are interested in how to make the patient experience and clinical investment more efficient and effective.
The purpose of the workshop is to discuss and plan a pilot project to develop and trial a patient-centred service model for people with these co-morbidities, and which will address the concerns expressed at our Darling Harbour meeting* regarding communication across services and sectors. The project will also include a research component to provide the basais for the service model. We are also expecting that the workshop will assist us in setting up a Taskforce Group to oversee the details and implementation of the pilot project.
*Background: in March this year, the Global Health Institute took advantage of the Diversity in Health Conference at Darling Harbour Convention Centre to hold an informal meeting to discuss issues regarding system and service responses to people with a mental illness and co-morbidities.
